2023 TSE Sports Camp Application - Jones-Onslow Electric Membership Corporation

I do hereby allow my child to attend the Carolina Basketball Camp, should he be chosen as a Jones-Onslow EMC Touchstone Energy Camper
I do hereby commit to provide transportation for my child to and from the Carolina Basketball Camp, located at UNC Chapel Hill, North Carolina June 17-21.
I do hereby allow my child to attend the Women's NC State Basketball Camp, should she be chosen as a Jones-Onslow EMC Touchstone Energy Camper
I do hereby commit to provide transportation for my child to and from the NC State Women's Basketball Camp, located in Raleigh, North Carolina on June 18-22 .
I understand that if my child cannot meet these commitments he/she will not be eligible for the scholarship.
I understand that the camp may implement protocols and/or adjust eligibility requirements related to COVID-19.
I understand that my child must comply with any pandemic-related protocols and requirements in order to participate.
